Your membership will remain active through your requested cancellation date, provided it is at least 30 days from the date this form is submitted. If your requested date falls within 30 days of submission, your cancellation will take effect 30 days from today instead.
Any payments scheduled during the 30-day notice period will be processed as normal. All payments are non-refundable.
If you choose to rejoin in the future, membership rates in effect at the time of reactivation will apply.
